Do You Need a Referral for Physiotherapy in Ontario?

Physiotherapist treating a patient during a physiotherapy session

No — you do not need a doctor’s referral to book physiotherapy in Ontario. Physiotherapists are direct-access practitioners, which means you can book an assessment directly, without seeing your family doctor first.

That direct access is the starting point, not the whole picture. Some extended health plans ask for a physician referral before they will reimburse a claim, and a few situations — such as a workplace injury or a motor-vehicle accident claim — can involve extra paperwork. It is worth knowing where you stand before your first visit.

What direct access means in Ontario

Direct access means a physiotherapist can assess you, identify the problem, and start a treatment plan without a referral from another health professional. In Ontario, physiotherapy is a regulated health profession, and registered physiotherapists are trained to assess musculoskeletal problems and to recognize when a problem needs a physician’s attention instead. At your first visit, your physiotherapist will take a history, assess how you move, and test strength, flexibility, and function. From that assessment, you get a plan — what the likely problem is, what treatment involves, and how many visits may be needed. If your assessment points to something that needs medical investigation, your physiotherapist will tell you and suggest you see your doctor. Booking directly also saves time. Rather than waiting for a doctor’s appointment to get a referral, you can book an assessment and start addressing the problem while it is still recent. Direct access also applies when you are returning for a new problem. You do not need a fresh referral each time you book, unless your insurer asks for one. If you are unsure, the clinic can help you check what your plan requires when you call to book.

When a referral may still be needed

The main exception is insurance. Some extended health plans require a physician referral before they will reimburse physiotherapy claims. The requirement varies by plan — some plans never ask for one, some ask for one only the first time, and some ask for one every plan year. If you plan to claim your visits, check your plan booklet or call your insurer before booking, so you know what your insurer needs on a receipt and whether a referral is part of it. Workplace and motor-vehicle claims can also involve a referral or a claim form. If your injury happened at work, your WSIB claim may involve your employer and your doctor alongside your physiotherapist. If it happened in a motor-vehicle accident, your auto insurer will have its own claim process and forms. In both cases, tell the clinic how the injury happened when you book, so the right paperwork can be started. A referral can also be helpful even when it is not required. If your doctor has already ordered imaging or lab work, bringing those results to your first visit gives your physiotherapist more to work with.

Frequently asked questions

Can I book physiotherapy without seeing my doctor first?

Yes. In Ontario, you can book a physiotherapy assessment directly, without a doctor’s referral. Your physiotherapist will assess you and tell you if your problem needs a physician’s attention.

Will my insurance cover physiotherapy without a referral?

It depends on your plan. Many extended health plans cover physiotherapy without a referral, but some require a physician referral before they will reimburse a claim. Check your plan booklet or call your insurer to confirm what your plan requires.

Do I need a referral for physiotherapy after a car accident or workplace injury?

Not always, but these claims often involve extra paperwork — a claim form, or a referral from your doctor. Tell the clinic how the injury happened when you book, and check with your insurer or case manager about what is needed.
If you are dealing with pain or an injury, you do not have to wait for a referral to get started. Book Now or call ISOMATIC at 647-873-7556.
