Physiotherapy is important for patients with respiratory conditions and Chronic Obstructive Pulmonary Disease (COPD). COPD is a broad term used to describe progressive lung conditions, including emphysema, chronic bronchitis, and refractory asthma. These conditions are characterized by increased shortness of breath, frequent coughing, wheezing, and tightness in the chest.

Causes of COPD

Cigarette smoking is the leading cause of COPD, though a small portion of cases (3-11%) occurs in non-smokers. Additional risk factors include exposure to second-hand smoke, outdoor air pollution, workplace dust and chemicals, fumes from burning fuels, and genetic predisposition. Inflammation and lung damage are triggered by exposure to these factors, contributing to COPD. Rising smoking rates worldwide, early diagnosis, and an aging population have increased COPD incidence.

Diagnosis and Classification

Diagnosis relies on symptoms, lung function tests (spirometry), and other tests like chest X-ray, CT scan, and arterial blood gas. The Global Initiative for Chronic Obstructive Lung Disease (GOLD) 2017 report classifies COPD into four grades based on the forced expiratory volume in one second (FEV1) values, from mild (GOLD 1) to very severe (GOLD 4). COPD is also categorized into groups A to D based on symptoms and exacerbation risk. Group D represents the highest risk.

Physiotherapy Management

Physiotherapy aims to reduce symptoms, promote independence and physical function, and prevent exacerbations. It includes clearing mucus from the lungs, exercising to strengthen the heart and muscles, and providing education to manage the disease. Physiotherapists also offer guidance on exercise, physical activity, breathing, and clearing mucus.

Pulmonary Rehabilitation Programs

Pulmonary rehabilitation, comprising supervised group exercise training and education, is an effective approach to enhance exercise capacity, minimize hospitalizations, and improve quality of life for patients. Exercise training is vital, including aerobic, strength, and flexibility exercises. It helps patients learn about exercise and disease management. Physiotherapists measure exercise capacity, symptoms, and safety for exercises and teach patients how to perform exercises. Education covers understanding the disease, symptom management, medication use, smoking cessation, and a healthy diet. These programs are available at hospitals and community centers across Australia.

Exercise Guidelines

For those with COPD, the recommended exercise is 30 minutes of aerobic activity on at least five days weekly, along with muscle strength exercises at least twice a week. Supervised exercise is preferable to unsupervised. Aerobic activities may include walking, swimming, cycling, or running. Strength exercises target major muscle groups, while flexibility exercises improve posture and reduce stiffness.

Safety of Exercise

Exercise is safe and beneficial for individuals with COPD and does not exacerbate the disease. It enhances quality of life, decreases symptoms, improves fitness, reduces depression, and lowers the risk of hospital admissions and death. Exercise is well-tolerated, with no reported serious adverse effects.
